Chronic Care Management

Chronic Care Management is a very important part of St. Luke’s Medical Practice and how Dr. Deucher partners with you to keep you as healthy as possible.

If you have Medicare/Medicare Advantage Plan and two or more chronic illnesses, Dr. Deucher will ask you to be enrolled in Chronic Care Management.

Once enrolled, Chronic Care Management is ongoing, proactive and covered by Medicare. It is how Dr Deucher checks in on you, follows you, and coordinates your medical care between appointments so chronic disease doesn’t get the upper hand.
